CLIENT CONSENT FORM

Nurture Mindful Healing offers a safe and supportive space for you to explore your concerns, gain a deeper understanding of yourself, and develop personal and interpersonal coping strategies. Counselling is a collaborative process between you and a trained therapist, working together to help you navigate challenges and work towards your personal goals.

Counselling may involve sharing sensitive and personal information, which at times can be distressing. It is natural to experience moments of increased anxiety or uncertainty during the process. While counselling often leads to positive change, outcomes vary for each individual. Your therapist is here to support you through every step of your healing journey.

CONFIDENTIALITY

All interactions with Nurture Mindful Healing, including scheduling, attendance at appointments, session content, progress in counselling, and client records, are strictly confidential. No information will be shared without your consent. You may request, in writing, the release of specific information to persons you designate.

Client records are securely stored in a password-protected system. You have the right to access your file under the provisions of the Freedom of Information Act, subject to certain legal exemptions. Your information is collected and stored in accordance with the Victorian Government Privacy Act 2000 and the Health Records Act 2001.

EXCEPTIONS TO CONFIDENTIALITY

Confidentiality is a fundamental part of counselling. However, there are legal and ethical circumstances where your therapist is required to disclose information:

  • If there is clear and imminent risk of harm to yourself or others, your therapist is legally obligated to report this to the relevant authorities.

  • If there is reasonable suspicion or disclosure of physical, emotional, or sexual abuse or neglect of a person under the age of 18, a report must be made to Child Protection Services.

  • Your therapist may seek professional supervision to ensure the best possible care. These discussions maintain confidentiality and are used for professional development purposes.

  • A court order, issued by a judge, may require the release of information contained in records and/or require your therapist to testify in legal proceedings.
